April Suzanne De Vera: Championing Public Health and Advocacy for Children and Youth with Special Healthcare Needs
Dedicated Public Health Nurse and Case Manager Makes a Lasting Impact in Camden County, New Jersey.
April Suzanne DeVera, a dedicated Public Health Nurse with the U.S. Department of Health and Human Services in Camden County, New Jersey, is making significant strides in advocating for children and youth from birth until their 22nd birthday with special healthcare needs or at risk for developmental delay. As a Case Manager in the Special Child Health Services Department, April works tirelessly to ensure that these children receive the support and resources they require to thrive. She makes referrals to and/or assists families to access state, federal, and community resources, which may include NJ Family Care, Early Intervention Services, Social Security Income (SSI), local charitable organizations, and Perform Care NJ Children System of Care. She collaborates with the child’s parents, guardians, physician, and specialists to evaluate the child’s strengths and needs. She assists youth and their families in the process of transitioning to adulthood. The special healthcare needs include, but are not limited to, long-term medical conditions, autism, physical disabilities, birth defects, developmental disabilities, and traumatic brain injury.
A graduate of Rutgers University – Camden with a Bachelor’s Degree in Finance and the University of Medicine and Dentistry of New Jersey with a Bachelor of Science in Nursing, April has combined her diverse educational background to create a unique approach to healthcare. Since 2013, she has co-owned and managed Momentum Medicine Plus in Voorhees, New Jersey, where she collaborates with the state to conduct consultative exams for Social Security Disability (SSD) and Supplemental Security Income (SSI), further demonstrating her commitment to supporting vulnerable populations.
April’s dedication to service extends beyond her clinical responsibilities. She actively engages with various case management groups, school groups, and community organizations, ensuring that families are informed and empowered to navigate the complexities of healthcare. Her passion for education and community involvement is evident through her published articles, local lectures, teaching engagements, and volunteer efforts within schools.
