Dr. Xequiel Hernandez, an esteemed family medicine provider, continues to provide exceptional care to the community at the Mount Sinai-Harlem Health Center. In addition to his family medicine practice, he has the added responsibilities as Medical Director at the Harlem Health Center and the LGBTQIA+ Fellowship Director at Mount Sinai. Since beginning his practice in 2012, Dr. Hernandez has established himself as a vital healthcare provider dedicated to improving the health and well-being of his patients in Harlem and the surrounding areas. In his endeavors to gain skills to improve the health of his patients, he has gained certification as an HIV specialist, Obesity Board for weight management, and Leadership training for physicians in administrative roles. 

Dr. Hernandez completed his undergraduate studies at Seton Hall University before earning his medical degree from Robert Wood Johnson Medical School. Following his graduation, he honed his clinical skills during his residency in Family Medicine at Hunterdon Medical Center. His commitment to patient-centered care is underscored by his certification from the American Board of Family Medicine.

Fluent in medical Spanish, Dr. Hernandez ensures that language barriers do not hinder patients from receiving high-quality healthcare. His ability to communicate effectively with diverse populations enhances the patient experience and fosters a trusting environment, essential for quality care.

In addition to his clinical work, Dr. Hernandez is actively involved in professional organizations that advocate for health equity. He is a proud member of the American Academy of Family Physicians and GLMA, an organization committed to advancing LGBTQ equality in healthcare. Dr. Hernandez plays a significant role in several medical committees, including the Medical Marijuana Task Force and the Addiction Committee, where he contributes to the development of policies and practices that improve patient care and access to necessary treatments.

His affiliations with prominent hospitals, including Mount Sinai Morningside, Mount Sinai Queens, The Mount Sinai Hospital, and Mount Sinai West, further enhance his ability to provide comprehensive and coordinated care to his patients.

Whether it’s discussing diabetes, heart disease, or the complexities of substance use disorder, I strive to empower patients with knowledge and resources.”

In addition to his clinical work, Dr. Hernandez is a passionate educator who has spoken at numerous town hall meetings, providing valuable insights into critical health topics, including prostate cancer awareness and sports medicine. Currently, as LGBTQIA+ Fellowship Director, he is working with teams to further develop the curriculum for teaching future providers to provide care for at-risk populations. In parallel, running quality improvement projects for Diabetic eye exams, Hypertension management with remote monitoring services, Colonoscopy screening, and Behavioral health screening efforts. His commitment to public health education underscores his dedication to improving health outcomes in the community.

Outside of his professional endeavors, Dr. Hernandez enjoys a variety of hobbies, including Mixed Martial Arts (MMA), rugby, wrestling, Aikido, and bodybuilding. He often finds inspiration for his work through his personal interests, emphasizing the importance of physical fitness and mental resilience.
